Our Transitions Specialists are essential to what we do. In this role you will facilitate transitions to assisted living and provide resources, support, and guidance for patients and clients who are failing at home or transitioning out of the healthcare system.
Transitions are the foundation of what we do at SilverStay. The Transitions Manager will guide and coach our team of Transitions Specialists from initial client contact through post-placement data collection. The Transitions Manager will ensure that our Transition Specialists are providing the highest quality of customer service by building processes and strategies that continually elevate SilverStay’s level of service.
COORDINATOR OF COMMUNITY SERVICES
The Coordinator of Community Services work with clients & their families, hospitals, assisted living operators, and Guardians to manage patients in the community with the goal of transitioning them from a health system contract to a safe care setting supported by the appropriate entitlement benefits. We're looking for someone who is highly organized, process-oriented, and enjoys seeking out resources and information to serve our clients and partners.
