This role is ideal for a motivated, outcomes-driven professional who thrives in fast-paced environments and wants the autonomy, creativity, and oversight to improve how patient discharges are handled.
Weekend Case Manager - Discharge Focused
As a Weekend Case Manager, you’ll be on the front lines of hospital discharge innovation. Working Saturday and Sunday on-site at one of our partner hospitals, you’ll lead the charge in keeping weekend discharges moving — reducing Monday backlogs, supporting staff, and maintaining momentum toward hospital throughput goals.

The Complex Case Manager will be embedded in one of our partner hospitals in Silver Spring, MD and play a key role in making complex discharges happen smoothly, quickly, and compassionately.
