Case Manager - Complex Cases
We are seeking Case Managers (LCSWs, MSWs, & RNs) to manage complex discharges with two partner hospitals; splitting time between Annapolis and Greenbelt, MD.
This role is ideal for a motivated, outcomes-driven professional who thrives in fast-paced environments and wants the flexibility, creativity, and autonomy to improve how patient discharges are handled.
Weekend Case Manager - Discharge Focused
As a Weekend Case Manager, you’ll be on the front lines of hospital discharge innovation. Working Saturday and Sunday on-site at one of our partner hospitals, you’ll lead the charge in keeping weekend discharges moving — reducing Monday backlogs, supporting staff, and maintaining momentum toward hospital throughput goals.
Registered Nurse Complex Case Manager
The RN Complex Case Manager will be embedded in one of our Baltimore hospital partners and play a key role in making complex discharges happen smoothly, quickly, and compassionately. By working directly with patients, families, and providers, and collaborating with SilverStay leadership, this role helps create scalable solutions that are reshaping how hospitals handle their most challenging transitions.
Software Engineer - AI Operations and Platform Development
We are hiring a Software Engineer to step into a dual-focus role split roughly evenly between platform development and AI operations.
On the AI automation side, they will inherit and expand a set of internal workflows, already in development, that use AI to eliminate repetitive, time-consuming work for the internal team: generating call summaries, identifying facility matches, drafting referral and family communications, and producing contract patient summaries.
Senior Account Executive - Health System Partnerships
SilverStay is hiring a Senior Account Executive to own and grow strategic hospital partnerships. This is a highly embedded, high-impact role for a former Director of Case Management, Care Management, or Patient Flow leader who also has strong commercial instincts.
This person will be onsite in hospitals every day—assisting with onboarding, managing operational and staffing challenges, building trust with clinical and executive leadership, and expanding pilots into long-term contracts.
Care Coordinator (Remote)
The Care Coordinator is the first person most people will connect with when they reach out to SilverStay. This is a high-impact, relationship-driven role for someone who communicates with warmth and clarity, moves quickly, and takes pride in being the calm, capable voice at the start of every client journey.
