Our Transitions Specialists are essential to what we do. In this role you will facilitate transitions to assisted living and provide resources, support, and guidance for patients and clients who are failing at home or transitioning out of the healthcare system.



The Coordinator of Community Services work with clients & their families, hospitals, assisted living operators, and Guardians to manage patients in the community with the goal of transitioning them from a health system contract to a safe care setting supported by the appropriate entitlement benefits. We're looking for someone who is highly organized, process-oriented, and enjoys seeking out resources and information to serve our clients and partners.



Answering Inbound Client Calls

The SilverStay Client Champion is the first person our clients will come into contact with when they call. We are seeking a candidate who will “wow” them from that very first conversation.
